In Minnesota the absentee ballot and absentee application for a ballot are two different things. It sounds like you are pretty well versed and are probably looking for where to send the actual ballot.You need to send your absentee ballot to your specific county auditor. This will be the county where you last lived. This website has a list of all county auditors, addresses, phone numbers, etc:
