I just transferred into Cornell University, though I was accepted right out of high school under a program they had called a Guaranteed Transfer. That is a different story though. I am not sure about the other schools, so I will be specific to Cornell.I would say you have a chance because you do have several extracurriculars that set you apart from the rest of your school, as well has have the grades they are looking for. But again, it's unpredictable.
The big thing with Cornell is who you are as a person, and how you fit into the school. This is more important than your grades. They are willing to forgive a few bad grades (though, I don't see that being an issue) if you seem like an interesting person who cares about education etc. and seem to fit into their campus.
Their second big thing is improvement. If you can show them, that you have improved, which you certainly have from a 2.5 to a 3.7, then they will be more willing to give you a shot.
I know a student from a community college in my area was accepted after going there for two years (its been all over the paper, and on their big LED sign "One of our students accepted to Cornell University!" so maybe you'lll make the paper! :D).
Personal advise on what you can do to better your chances, is to look at what they would have wanted one of their 4 year students to have completed by their junior year and make sure you have those classes done so that you aren't behind once there. I would also call admissions for which ever of the colleges in Cornell you are going to apply to, talk to them, and try and set up an interview. Maybe try to connect with some local Alumni.

OH last thing!
I haven't checked out any of the apps for transferring, but generally speaking, the essay is the most important part. Tell a story that gets them to know who you are. Apart from that your resume looks good, so get at it!
