Wow, that sounds really nice. A slideshow would be awesome. Maybe you could add some pictures of you and her together to it as well. Make the font her favorite color and add some pictures of her favorite things along with reasons why she is the best mom in the world to you. Also, a good title of the slideshow could be "Why I love mom..."

Another option could be a hand-made photo album with reasons of why you love her next to diffrent pictures on each page.
