A name change in PAN card requires an explanation in the first place why such a change is needed.

Father's name and date of birth are the other two important data on the card. Address proof is also required which can be supported by a Ration Card. (SSC will not do!).

If the parent's name and date of birth have not changed, then, the procedure for a NEW pan card will have to be followed after surrendering the old card. This is so because when the name was given as RAJEEV KUMAR, the number allotted would have been XXXPKXXXXX. If the surname is to be changed to Shrivastav, then the number would become XXXPSXXXXX.

You may approach the concerned PAN issuing organisation like UTITSL or NSDL for clarification and help.

In india, the system & practice in general administration in all departments of govt is to dragg a common man to difficult situations. so is adopted, in your case, while disposing off, your passport application. My sugestion would be to complete their difficult formalities and obtain the passport. Latter, pl don't spare the responsible officer for compelling you to choose options as per his sweet will. File a consumer dispute case latter, impleading that man in person & also their office.

It is the government that will issue you a passport, so all the documents listed, out of which you may produce any two for applying a passport MUST have been issued by the government.

Baptismal Certificates are not a government document, and so it is not acceptable to the passport authorities.
