Go to your city hall, they have voter registration forms there. In most states you can register when you get you driver's license. You can also register through the websites for the three main presidential candidates.

You can definitely go back to where you got the registration for a copy of it. In this case you say it is your DMV, so I would go back there first. This seems to be the easiest simplest way.

If you need to do something else they would also know the exact right thing to do. No guessing and no figuring out.

Depends. If you are not registered to vote in Virginia and you voted in the Virginia races then yes, unfortunately that is illegal. If you voted for Arkansas state races then you have obeyed the law, it is called an absentee ballot

Since you only committed a misdemeanor, you should be fine. The states that revoke your voting rights do so for felonies.

Only 3 states impose a life-long denial of the right to vote to all citizens with a felony record, and 9 others do for a various length of time.
