You can get your I Card rectified by depositing it in the Office of the ERO concerned or at Photography Centre when the work of preparation of Photo ID card begins.[Election Commission of India - Electoral rolls - Q.12 of FAQs]
You have to apply in FORM ID ECI-EPIC-004 of the Election Commission of India, meant for Application for Rectification of Defects in Elector’s Photoidentity Card (EPIC). The application should be addressed to The Electoral Registration Officer of your concerned Assembly Constituency.

Go to your nearest electoral office alongwith a photo, Residential proof and identity proof. Just fill up the form and you have done with that. It is hassle free.Some times they may take a photograph then and there. In that case a photograph is not necessary.
