Not everyone has ID (not everyone drives).ID cards can be lost or stolen.
People travel and may be away from home on election day, requiring absentee ballots that have to be mailed in, so there's no instant feedback of putting the ballots into a scanner.
Computers make mistakes in reading ballots.
People have a right not skip voting for an office, so a section of a ballot might be unmarked on purpose.
Election officials can make errors in judgment, for example they might not think someone who changed their hair color and style looks like their ID, requiring provisional ballots.
Not every place uses optical scan ballots.
A properly registered voter may not be on the list at the polling place (I've been an official poll watcher and seen this happen, a call to the county is required to correct this, but the person has to be given a provisional ballot since there's no way to sign them in).
As simple as the process seems, mistakes and complications can and do happen. Humans are part of the process of voting (they're the ones checking people in and voting) , and that unavoidably complicates the process a little. We hear a lot about the problems, but little to nothing about the vast majority of voters and precincts that enjoy a smooth and pleasant experience. There's always room for improvement, but on balance, we should consider ourselves fortunate that the process works as well as it does.
