Basic documents required are only two i.e. proof of date of birth and proof of residence. If you are born after 1.1.1989, then only birth certificate is accepted at proof of date of birth. Else, copy of SSC certificate will suffice. For residence, you can give a copy of telephone/electricity bill, bank passbook etc. Thereafter, depending upon your status (whether minor, student or employed) you may have to produce more documents. Like if you are employed with govt/PSU, you are required to submit an NOC from employer. If student, then a certificate from institution etc. For details, visit www.passport.nic.in

IF you were in the USA, we would tell you to go to your local (usually county) Probate Court. There, you can ask the Clerks for the forms to fill out. (Take some money to cover court costs.) Then a hearing before the Judge will be scheduled. THE #1 most important thing is that you are NOT changing your name to avoid creditors. The Judge will ask you some questions.

It is an easy process - you do not need an attorney for this.
